SAO PAULO/BRASILIA, July 23 - Brazil's official accident investigation authority has released its final analysis of the August 9, 2024 crash of Voepass Flight 2283, concluding that a constellation of technical failures, weather-driven icing and operational shortcomings at the regional airline all contributed to the fatal accident that killed 62 people.

The aircraft, an ATR 72-500, departed Cascavel in southern Brazil bound for Guarulhos international airport in Sao Paulo. It lost control over the town of Vinhedo, roughly 80 km northwest of Sao Paulo, and entered a stalled, rotating descent often described by observers as a "flat spin" before impacting the ground.

In its final report, the Center for Investigation and Prevention of Aeronautical Accidents (Cenipa) said investigators found the plane had encountered atmospheric conditions conducive to severe icing and that the aircraft's de-icing system suffered several failures. The crew had activated and deactivated the de-icing system multiple times while receiving warnings related to icing.

Lieutenant Colonel Paulo Mendes Froes, speaking for the investigative authority, said the report lists 19 factors that together contributed to the accident. Those factors include the flight crew's emotional state, external influences, adverse weather conditions and shortcomings in regulatory oversight.

A preliminary investigation released earlier in 2024 had already documented signs of ice accumulation on the airframe but stopped short of identifying a single definitive cause, noting that aviation accidents typically result from multiple interacting issues.


Crew response and cockpit warnings

Cenipa reported that crew members did not follow required procedures after eight separate "cruise speed low" warnings sounded during the flight, and that they did not take the appropriate actions following a "degraded performance" alert even though they discussed visible ice accumulating on the aircraft.

According to cockpit recordings cited in the report, the captain remarked that the de-icing system appeared to be malfunctioning. At one point the first officer is recorded saying he had forgotten to activate the system. Cenipa suggested these lapses may have been influenced by cognitive phenomena known as "inattentional blindness" and "inattentional deafness," where high workload can cause crews to miss critical cues.


Maintenance practices and organizational culture at Voepass

Investigators also examined broader operational issues at Voepass. Cenipa described a "normalization of deviations" within the airline, finding that earlier failures of the same de-icing system had not been properly logged in maintenance records. The report further cites evidence that certain maintenance tasks were carried out without required supervision.

Froes said this pattern reflected a culture of informality in which pilots and mechanics sometimes omitted recording aircraft-related issues in maintenance logs, apparently to allow aircraft to return to service the next day. That culture, Cenipa said, contributed to the accumulation of unresolved technical problems.

The investigation additionally concluded the aircraft should not have been dispatched at departure because of a windshield wiper fault, although it noted that the wiper issue did not directly cause the accident.


Manufacturer, regulator and industry recommendations

Cenipa pointed to prior incidents worldwide involving ATR 72 aircraft where ice accumulation preceded loss of control and recommended that the European Union Aviation Safety Agency (EASA) coordinate with ATR to review the manufacturer's "degraded performance" procedure. The report described that review as a measure intended to reduce the likelihood of crews encountering comparable limit conditions in future flights.

The report notes that ATR is a joint venture between Europe's Airbus and Italy's Leonardo. The document states ATR and EASA did not immediately provide responses to requests for comment.

Operationally, the report's findings have already had regulatory and commercial consequences. Brazil's aviation regulator ANAC suspended Voepass operations citing safety concerns, and Voepass subsequently filed for bankruptcy protection in April 2025, one month after the suspension.

ANAC has said it will carry out a technical analysis of Cenipa's findings and of the recommendations that fall under the regulator's authority. That review is expected to take up to 120 days.


The Cenipa final report frames the crash as the result of a complex interaction among degraded aircraft performance in icing conditions, repeated technical failures of a critical anti-ice system, lapses in cockpit procedure and systemic maintenance and oversight weaknesses at the airline.
