Death Toll Across the Middle East Rises as U.S.-Israeli Strikes on Iran Trigger Widespread Retaliation

Since U.S. and Israeli strikes on Iran on February 28, a cascade of Iranian attacks on Israel, U.S. bases and Gulf states - and a new front in Lebanon - has produced thousands of deaths across the region. Reported totals vary by source and Reuters has not independently verified the figures. The counts include civilians, combatants, and military personnel from multiple countries and show the conflict’s wide geographic spread.

Key Points

  • Multiple sources report thousands of deaths across the Middle East following U.S.-Israeli strikes on Iran on February 28, with HRANA and the IFRC providing differing tallies for Iran.
  • The conflict has produced casualties among civilians, fighters and military personnel across several countries, including Iran, Lebanon, Iraq, Israel, the United States and Gulf states - impacting defence, maritime and logistics operations in the region.
  • Reported incidents include missile and drone strikes, a military aircraft crash, attacks on tankers near ports, and a helicopter crash, illustrating a range of attack vectors that have produced civilian and service member fatalities.

The conflict that followed U.S. and Israeli strikes on Iran on February 28 has produced large numbers of casualties across the Middle East, according to multiple reports. The strikes precipitated a series of Iranian retaliatory attacks on Israel, on U.S. bases, and on states in the Gulf, while also opening a new front in Lebanon. Reported death tolls differ by source and have not been independently verified by Reuters.

Iran

A U.S.-based rights group, HRANA, reported on Friday that 3,389 people have been killed since the outbreak of the conflict. HRANA said 1,527 of those killed were civilians, including at least 228 children. The organisation said its figures come from field reports, local contacts, medical and emergency personnel, civil society networks, open-source materials and official statements.

The International Federation of Red Cross and Red Crescent Societies (IFRC) reported on Friday that at least 1,900 people have been killed and 20,000 injured in Iran as a result of the U.S.-Israeli strikes. It was not clear from the IFRC statement whether its totals include at least 104 people the Iranian military said were killed in a U.S. attack on an Iranian warship off Sri Lanka on March 4.

Lebanon

Lebanese authorities put the death toll from Israeli strikes at 1,142 since March 2, and said that figure includes at least 122 children. Separately, two sources familiar with Hezbollah’s own accounting told Reuters that more than 400 fighters from the group have been killed since Hezbollah launched attacks at the outset of the new Lebanon front on March 2. It is unclear whether the Lebanese authorities’ tally includes the number of fighters reported by those familiar with Hezbollah’s count.

Iraq

Iraqi health authorities said at least 95 people have been killed since the start of the crisis. That total encompasses civilians, members of the Iran-affiliated Shi’ite Popular Mobilisation Forces, U.S.-allied Kurdish Peshmerga fighters, and soldiers in the Iraqi army. Port security officials also reported that one foreign crew member was killed in an attack on tankers near an Iraqi port.

Israel

Israel’s ambulance service said missiles launched from Iran and Lebanon towards Israel have killed 19 people inside Israel. The Israeli military reported that four of its soldiers were also killed in southern Lebanon. In a separate incident, Israeli forces misfired and killed an Israeli farmer near the Lebanese border on March 22.

United States

Thirteen U.S. service members have been killed during operations linked to the crisis. Six were confirmed dead after a U.S. military refuelling aircraft crashed over Iraq, the U.S. military said, while seven others have been killed in action during operations against Iran. U.S. officials told Reuters that twelve U.S. troops were wounded - two seriously - in an Iranian military strike on Prince Sultan Air Base in Saudi Arabia.

United Arab Emirates

The UAE reported ten deaths in Iranian attacks, including two army soldiers, according to UAE authorities.

Qatar

Qatar’s defence ministry said seven people were killed on March 22 in a helicopter crash in Qatari territorial waters after a technical malfunction during what it described as "routine duty." The ministry gave limited details, saying four of those killed were members of the Qatari armed forces, one was a Turkish serviceman attached to the Qatar-Turkey joint forces, and two were technicians working for Turkey’s defence firm ASELSAN.

Kuwait

Kuwaiti authorities reported six deaths. Those included two people killed in Iranian attacks, two interior ministry officers, and two army soldiers.

West Bank

Four Palestinian women were killed in an Iranian missile attack in the Israeli-occupied West Bank, authorities reported.

Syria

State news agency SANA reported that four people were killed when an Iranian missile struck a building in the southern city of Sweida on February 28.

Bahrain

Bahraini authorities said two people were killed in two separate Iranian attacks, the most recent of which struck a residential building in the capital, Manama. The UAE’s defence ministry reported on March 24 that one of its civilian contractors was killed in an Iranian attack on Bahrain, identifying the contractor as a Moroccan national.

Oman

Omani authorities reported two people killed on March 13 in a drone strike on an industrial zone in Sohar province - the first fatalities recorded on Omani soil in this crisis, according to the reporting. Separately, the manager of a vessel said one person had previously died when a projectile struck a tanker off the coast of Muscat.

Saudi Arabia

Saudi authorities reported two people killed when a projectile fell on a residential location in Al-Kharj city, southeast of the capital, Riyadh.

France

France said one of its soldiers was killed and six others wounded in a drone attack in northern Iraq while they were providing counter-terrorism training.

The casualty figures span combatants and civilians, including children, and cover a range of incidents from missile strikes and drone attacks to a military aircraft crash and maritime incidents affecting tankers. The counts are drawn from national authorities, rights organisations and humanitarian agencies, each using their own reporting methods and sources. Given the differing methodologies and the ongoing nature of the conflict, the totals may be subject to revision as further information becomes available.

Risks

  • Ongoing hostilities and retaliatory strikes pose continued operational risks to maritime shipping and port activities, as evidenced by attacks on tankers near an Iraqi port - impacting shipping and logistics sectors.
  • Military bases and service members remain exposed to further attacks, highlighted by casualties from a refuelling aircraft crash over Iraq and wounded troops at Prince Sultan Air Base, which could affect defence deployments and allied operations.
  • Discrepancies and evolving counts across multiple reporting organisations and official sources create uncertainty around the scale and distribution of casualties, complicating humanitarian response planning and market assessments tied to regional stability.
